Fayette County Public Health recently released the following restaurant inspections:

August 9

Demented Bar & Grill, 618 Rose Ave. Follow up inspection. Violations/Comments: the cleanliness of the kitchen has now improved and is now satisfactory. The person in charge stated that the facility is now scheduled for extermination and the ice machine is now fixed, however water was still visible on floor.

Cafe YUSA, 151 Jamison Road South West. Follow up inspection. Violations/Comments: All food are now reading at safe temperatures. The salad station is now at 41 degrees Fahrenheit and below. The ware washing sanitizing solution concentration is now correct. Thank you. Also according to management, two employees and food handlers will be taking serve safe courses within a week.

Carriage Court WCH, 500 Glenn Avenue. Standard, complaint and Critical Control Point (FSO) inspections. Violations/Comments: food safety certifications were unavailable. The person in charge must ensure that employees are properly trained in food safety. Dented cans were found inside the food pantry. Food packages must remain in good condition to protect food from contaminates. Beef (raw) in the thawing process was found stored next to deli meat inside of the preparation cooler. Raw and ready-to-eat food must be separated to prevent contamination. The meats were separated immediately. The microwave was fund with food residue accumulation as well as the kitchen counters, utensil holder (in smaller kitchen) and kitchen walls. Food and non food contact surfaces must be clean to sight and touch. Vegetables were found stored inside of the preparation cooler past seven days (chopped vegetables). Perishable food items must be discarded within seven days. These food items were removed and discarded immediately. Disposable utensils were found stored under the hand sink in breakfast bar area. Food equipment and utensils must be stores in a clean location where it is not exposed to any sources of contamination. These were removed immediately. Knowledge and Responsibility: The person in charge did not display good knowledge of Critical Control Points and several critical violations were present. A phone complaint was received on Aug. 8 concerning food served uncooked and cold. All temperatures were in the safety zone during the inspection. The person in charge was notified about the complaint and proper temperatures were discussed.

August 8

Roosters, 120 Crossings Drive. Standard and Critical Control Point (FSO) inspections. Violations/Comments: Several food containers were found with food residue located on the clean food equipment rack. Food contact surfaces must be cleaned to sight and touch. A few food containers were found cracked. Food and non food contact surfaces must be smooth and easily cleanable. Food residue and trash were found under the food equipment located in the main kitchen area. The facility must be cleaned as often as necessary to keep them clean. Knowledge and Responsibility: Management displayed good knowledge of CCP however critical violations were present.

Walmart Food Distribution Center #6089, 1400 Old Chillicothe Road. Standard and Critical Control Point (FSO) inspections. Violations/Comments: Utensils were found stored inside drawers with soil accumulation. Food contact surfaces and utensils must be stored in a clean location. The white freezer located inside of the storage room area has an accumulation of ice along walls and shelves. All equipment must be maintained in a state of good repair.

Demented Bar & Grill, 618 Rose Avenue. Follow up inspection. Violations/Comments: cleaning has improved in the kitchen, however it is not satisfactory. Re-inspection scheduled for Aug. 9.

BPO Elks 129, 277 South Main Street. Standard inspection. Violations/Comments: food safety certifications were unavailable The person in charge must ensure that employees are properly trained in food safety. Disposable cups and lids were found stored out of their plastic wrap inside of boxes located in cabinets near restrooms. Single use items must be maintained in the protective wrap or stored in a clean location. Covered waste receptacle were unavailable inside of the women’s restrooms. Covered waste receptacles must be provided for sanitary napkins disposal .

August 7

The Highland House, 783 State Route 41. Complaint inspection. Violations/Comments: a phone complaint was received on Aug. 5 regarding unsanitary kitchen conditions. The customer states that they were at the facility for an event on Aug. 3 and observed a dirty sliver, sticky tables and counters, dirty utensils and equipment, moldy food inside of the refrigerator and a dirty kitchen in general. The customer stated that they were concerned about someone becoming ill from these unsanitary conditions. The following areas or items were found with food residue or soil accumulation: knives, forks, spatulas, utensil holders, serving spoons, sliver, plastic food containers and cookie sheets were all found in main kitchen area. Also plates and bowls. Inside of the small kitchen a moldy ice machine was found. Food contact surfaces must be cleaned to sight and touch. Gnats were found near the ware washing sink, inside of the small kitchen. The presence of insects must be controlled to minimize their presence on the premises. These gnats were found swarming around a rotten lemon left inside of the sink. The following non food contact surfaces were also found with soil accumulation: kitchen (main) floors especially around the fryers and stove, counters, ware washing sinks, preparation cooler located in small kitchen. Non food contact surfaces must be cleaned as often as necessary to prevent accumulation of soil residue. Food equipment was found inoperable: Arctic Air Refrigerator (white), walk-in cooler, beverage air preparation cooler, true preparation cooler, Arctic Air freezer (located in small kitchen). All equipment must be maintained in a state of good repair. The equipment must be replaced or removed from the food establishment. Rotten lemons were found inside of the preparation cooler located in the small kitchen. Unsafe food must be discarded. Visibly soiled wiping cloths were found on counter near the ware wash machine. Cloths that are in use for wiping counters and other equipment must be held between uses in a chemical sanitizer solution and laundered daily.

Cafe YUSA, 151 Jamison Road South West. Standard and Critical Control Point (FSO) inspections. Violations/Comments: management food safety certification and food handler certifications were unavailable. The person in charge must ensure that employees are properly trained in food safety. Several food items on the salad bar station were above 41 degrees Fahrenheit. According to management and staff the food items were placed in the cold holding station 1.5 hours prior to the inspection and they did not have enough ice to keep the temperature down or below 41 degrees Fahrenheit. During the inspection, ice was obtained from a nearby ice machine and placed into the unit. The temperature did drop down some but not to 41 degrees Fahrenheit. according to management, the food items are in the serving stations from 10:30 a.m. to 12:45 p.m. and discarded at the end of the day. Considering the time frame that the food is out of safe temperature, less than four hours, time as a public health control procedures are an option for the facility. Management decided to use these procedures temperatures for the day. Management stated a block would be purchased this evening to ensure temperature. A follow up inspection is scheduled. The chemical sanitizing solution was too low. Bleach will be used in the sink until the other solution is corrected: ware washing sinks. Knowledge and Responsibility: the manager displayed good knowledge of Critical Control Point questionnaire however critical violations were present.

August 6

Baymont Inn & Suites, 11431 Allen Road, Jeffersonville. Standard inspection. Violations/Comments: An employee health notification policy was unavailable. Employees must be informed of their responsibility to report any food borne illnesses to their managing staff. A copy of the rules were provided. Disposable food utensils were found stored under the hand wash sink. All food and food equipment must be stored in a way and in a location where contamination is prevented. The utensils were removed and placed in a safe location immediately. The kitchen cabinets were found lined with newspaper. Food and non food contact surfaces must be smooth and easily cleanable. The newspaper was removed immediately. The following areas were found with soil accumulations: kitchen cabinets and drawers, three compartment sink (ware washing sink). Non food contact surfaces must be cleaned at a frequency necessary to keep them clean.

August 5

Demented Bar & Grill, 618 Rose Avenue. Standard inspection. Violations/Comments: the person in charge is not trained in food safety. The person in charge stated that they were in the process of obtaining a food safety certification. Plates and mugs found inside of the kitchen cabinets were found with food residue. Food contact surfaces must be cleaned to sight and touch. Mice droppings were found on the kitchen counters and floor. The presence of insects and pests must be controlled to minimize their presence on the premises by routinely cleaning, inspecting and using pest controlling devices. The following areas of items were found with an accumulation of soil residues: kitchen counters, refrigerator, microwave, fryer near storage room. The facility must be clean as often as necessary to keep them clean. A puddle of water was found under and around the ice machine All equipment must be maintained in a state of good repair.
