Arby’s #6297, Jeffersonville. Standard inspection. Violations/comments: Observed food debris under rack in dry food storage on the floor and wall, observed build up of grease on fryer hood and ANSEL equipment.

220 State Street, Inc, Jeffersonville. Standard inspection. Violations/comments: Observed tuna in cooler with no date mark observed other food (veggie beef) in small cooler with no date marking, observed cappuccino machine not clean to sight or touch, observed pop machine nozzles not clean to sight and touch.
SEPT. 15
Subway, 1377 Leesburg Avenue. Standard inspection. Violations/comments: Observed no test strips to test sanitizer.
Wal-Mart Super Center Subway, 1397 Leesburg Avenue. Standard inspection. Violations/comments: Observed mice droppings in cabinets, observed opened bag of meatballs in cooler without a date mark, observed pop machine to be dirty to sight and touch.
Subway Franchise, 8465 Factory Shops Blvd. Standard inspection. Violations/comments: No violations at time of inspection and very clean facility.
Subway 35/71, 12484 St. Rt. 35 NW. Standard inspection. Violations/comments: Observed pop machine to have debris on it and not clean to sight and touch.

Walmart #3251, 1397 Leesburg Avenue. Standard inspection. Violations/comments: Observed expired Gerber graduates Grabbers Banana, Pear and a zucchini baby food on shelf past the best by used date of Sept. 15, observed dry dog food on the floor underneath the dog food area.
BPO Elks 129, 277 S. Main Street. Standard inspection. Violations/comments: Observed no test strips for quat sanitizer.

D.B. Yummers, 1114 Bush Road SW. Standard inspection. Violations/comments: No violations noted at time of inspection.
American Legion #25, 1240 U.S. Rt. 22 NW. Standard inspection. Violations/comments: Observed wall by fryer with debris on it and is not smooth to the touch, observed beef patties in cooler and hot dogs in cooler without a date mark on them.
VFW 3762, 335 Veterans Way. Standard inspection. Violations/comments: Observed hood over fryer not clean to sight or touch.
Eagles/Wash C.H. Aerie #423, 320 Sycamore St. Standard inspection. Violations/comments: Observed dirt on vent in kitchen cooler.
Wendy’s, Jeffersonville. Standard inspection. Violations/comments: Observed food debris underneath fry machine and on the wall behind fry machine, observed items in cooler without a date marking on them, manager discarded tomatoes and broccoli salad.

McDonald’s Restaurant, Jeffersonville. Standard inspection. Violations/comments: Observed build up of debris on ice machine in the storage room, observed no test strips for the three compartment sink they need “quat” test strips not chlorine, observed no date mark on burritos and lettuce in cooler, foods can spoil if not served in time, observed back storage room and freezer with debris on floor and under racks.

Tim Hortons, 520 Clinton Avenue. Standard inspection. Violations/comments: Observed chicken in cooler without proper date mark, observed coffee dispenser to have debris under spout.
Rax WCH, 102 Washington Square. Standard inspection. Violations/comments: Observed chicken breast in walk-in cooler without a proper date mark, observed a build-up of grease underneath the three compartment sink as well as the fryer in front of store.
Demented Bar and Grill, 618 Rose Avenue. Standard inspection. Violations/comments: No violations noted at time of inspection.
Frisch’s of Washington C.H., 1830 Columbus Avenue. Standard inspection. Violations/comments: Observed build-up of debris underneath fryer in kitchen.

Jeffersonville TL, Jeffersonville. Standard inspection. Violations/comments: Observed build-up of debris on side of fryer and back of fryer, observed build up of debris underneath soda machine nozzle area.
Center Pizza, 23 Fayette Center. Standard inspection. Violations/comments: Observed “Quat” sanitizer tablets with chlorine test strips.
Pizza Hut #24171, 409 S. Elm St. Standard inspection. Violations/comments: Observed build up of grease debris on hood above fryer in back of restaurant.
Woody’s Cafe, 222 N. Main St. Standard inspection. Violations/comments: Observed food in refrigerator with no date marking (bacon, onions, cheese and cut tomatoes), observed grill area to have debris on it.
SEPT. 24
McDonald’s, 280 S. Elm St. Standard inspection. Violations/comments: Observed grease build up on the side of the main grill and underneath the main grill, entire floor, especially under equipment/racks need thoroughly cleaned, observed debris underneath pop machine nozzles are, observed debris on orange juice machine and creamer machine, observed debris in cooler and freezer underneath the rocks, observed build up of debris in microwave on ceiling of microwave, observed a build up of debris on both fryer areas, manager has started clean-up process, follow-up date is set for Oct. 1.
Ranchers Roast Beef, 501 S. Elm St. Standard inspection. Violations/comments: Observed debris on fryer hood, observed no cover on trash can in women’s restroom.
